What Are the Signs of Low Testosterone?

Have you been feeling more tired than usual, noticing your mood is off, or wondering where your drive has gone? You are not imagining it. For many men, these changes are not just “getting older.” They can be signs of low testosterone, a common and treatable condition. This article explains what low testosterone looks like, what causes it, and how a medically supervised approach can help.

Testosterone Does More Than Most People Realise

Testosterone is often thought of purely as a sex hormone, but its role in the body is much broader. It supports muscle mass, bone density, red blood cell production, mood regulation, and cognitive function. Levels typically peak in early adulthood and decline gradually from around age 30 onwards, at a rate of roughly 1% per year.

For some men, this decline is steep enough to cause noticeable symptoms. Clinicians refer to this as hypogonadism. Because the drop happens gradually, many men do not connect their symptoms to testosterone until several signs have built up over time.

The Physical Signs of Low Testosterone

Low testosterone often shows up in the body before it is recognised as a hormonal issue. Common physical signs include persistent fatigue that does not improve with rest, a loss of muscle mass or strength despite regular training, and an increase in body fat, particularly around the midsection.

Reduced bone density is another effect, which can raise the risk of fracture over time. Some men also notice thinning hair, reduced body or facial hair growth, or unexplained hot flashes and sweating. None of these signs on their own confirm low testosterone, but together they form a pattern worth investigating.

The Emotional and Cognitive Signs

Hormonal balance affects the brain as much as it affects the body. Men with low testosterone commonly report low mood, irritability, or symptoms that closely resemble depression. Concentration can suffer too, often described as a persistent “brain fog.”

Motivation and drive tend to decline alongside these changes, and sleep is frequently disrupted, whether through insomnia or generally poor sleep quality. These symptoms are easy to attribute to stress or lifestyle, which is part of why low testosterone often goes undiagnosed for years.

The Sexual Signs of Low Testosterone

For many men, the sexual symptoms are what finally prompt a conversation with a clinician. These include a reduced sex drive, difficulty achieving or maintaining erections, fewer spontaneous erections, and a noticeable drop in semen volume.

These changes can affect confidence and relationships as much as physical health, and they are among the clearest indicators that testosterone levels are worth checking.

What Causes Testosterone to Decline

Several factors can contribute to low testosterone beyond natural ageing. Obesity is a significant driver, as excess fat tissue can convert testosterone into oestrogen. Chronic conditions such as type 2 diabetes, kidney disease, and liver disease are also linked to lower levels, as is sleep apnoea, which disrupts the hormone production that normally occurs during deep sleep.

Certain medications, including opioids and some steroid treatments, can suppress testosterone, as can chronic stress through sustained cortisol elevation. Less commonly, testicular injury, infection, or disorders affecting the pituitary gland or hypothalamus can also be responsible.

What a Clinical Assessment Can Reveal

Because low testosterone symptoms overlap with conditions like depression, thyroid disorders, and chronic fatigue, self-diagnosis is unreliable. A proper assessment starts with a blood test, usually taken in the morning when testosterone levels are naturally at their highest.

At PrimeLife, this means a comprehensive symptom review, laboratory testing of total and free testosterone alongside related markers, and a one-on-one consultation with a clinician to review the results in the context of your health history. This clarity is often the first real answer many men get, after months or years of putting symptoms down to stress or age.

A Programme Built Around You

At PrimeLife, testosterone replacement therapy is a clinical programme, not a product. Every patient has a consultation with a qualified prescriber before treatment begins, and no medication is prescribed without a full assessment of the patient’s health picture. Where TRT is appropriate, the clinician prescribes and monitors it as part of an ongoing relationship, with regular reviews built into the process.

If your situation changes or your clinical needs evolve, your clinical team responds accordingly. You are not simply handed a prescription and left to manage alone. If you have been feeling less like yourself and are not sure why, the answer may be sitting in your bloodwork rather than your daily habits.
